I’m Buffy Schwery, RN, BSN. As your nurse delegator, I partner with you and your caregivers in adult family homes to ensure daily care is safe, simple and easy to follow. Together, we’ll create a clear plan so you can feel confident and supported every day.
Services Provided
RN Clinical Oversight
I personally provide one-on-one expert guidance and ongoing monitoring to help caregivers deliver safe and reliable care with confidence. I remain accessible for questions, reinforcement and clarification as needed, to ensure care continues to meet established standards.
Care Planning and Assessments
I offer detailed assessments and personalized care plans to ensure safety, promote consistency, and support high-quality care in a home care setting. Each plan is tailored to the client’s individual needs and clearly outlines tasks, expectations, and safety considerations. In addition, guidance and support is provided to caregivers to ensure care is delivered correctly and with confidence.
Medication Delegation
I identify which medication tasks are safe for caregivers to practice and administer. Once these have been identified, teaching is provided on how to safely deliver care. My goal is to ensure every caregiver feels fully trained and confident in supporting the health and care of the patient.

My clinical background includes over five years of experience supporting patients and caregivers in adult family homes through hospice care. In addition, I have a strong foundation in hospital nursing across cardiac, orthopedic, neurological, and medical-surgical units, including step-down ICU. I bring a practical understanding of complex care needs, medication safety, and caregiver support with a focus on making care clear and manageable in real-world settings.
